Description:

The Child and Teen Shot Clinic is held on the First Floor of the Portage County Administration Building (next to WIC Office). Walk-in only. Infants through age 20 may use the Child and Teen Shot Clinic.
Shots for adults are by appointment-only and must be scheduled through Nursing Services. Please call 330-298-4490 to make an appointment.
DIRECTIONS TO SHOT CLINIC:
· The Administrative Building is located between Chestnut and Meridian St.
· The Nursing Clinic is located near Chestnut St. There is convenient parking off of Chestnut St.
· The clinic is easily accessible from Rt. 59 as well as St. Rt. 44. For more information on directions please call 330-298-4490.
· Other Portage County Health Department Services are located on the 3rd floor of the Administrative Building.
